Yes, it really seems like it's up to the algorithm, and the amount of followers doesn't mean too much. I have around 4k followers, and I usually get 50-75 reads per day.

The only method I know for increasing reads is to focus on building relationships with your readers. Spending time supporting and interacting with them is also a great way to make new friends.

Anything else is a bonus on Medium because it's hard to predict if an article will reach anyone.
